Deadlines

Health permit documentation is due for undergraduate BSN students:

  • Continuing BSN Second Career/Degree (CD2) and RN to BSN students by April 15
  • Continuing Traditional BSN students by May 15
  • Newly admitted BSN Second Career/Degree (CD2) by June 15
  • Newly admitted Traditional BSN students by October 15

Documentation is due for all MSN, graduate certificate, and DNP students is due prior to the semester in clinical:

  • Fall August 15
  • Winter December 15
  • Spring/summer April 15

Notice of risk/declination and exemption forms

Religious exemptions are accepted for COVID and influenza ONLY.

Flu

Students who request a religious or medical exemption for the flu vaccination must complete the WSU Influenza Declination Form provided by the Campus Health Committee and upload the request to the university portal. Once approval is received, please upload the decision to your Castlebranch account under the flu requirement.

COVID

Students who request a religious or medical exemption for the COVID vaccination must complete the College of Nursing COVID Exemption Form and email the request to the Office of Academic and Clinical Affairs at condeclination@wayne.edu. Once approval is received, please upload the decision to your Castlebranch account under the COVID vaccination and COVID booster requirement.

All other immunizations and vaccinations

Religious exemptions are not allowed for MMR, Hep B, Varicella or Tdap. If a student is requesting an exemption for any of these requirements, only medical-related reasons will be considered. Students who are requesting a medical exemption for any of these requirements must complete the College of Nursing Declination Form and email the request to the Office of Academic and Clinical Affairs at condeclination@wayne.edu. Once approval is received, please upload the decision to your Castlebranch account under the respective requirement.

ACE requirements

Undergraduate and selected graduate students must meet requirements for the Alliance for the Clinical Experience (ACEMAPP). Students will receive an email with additional information about completing their profile and completing three learning modules: HIPAA, OSHA and Blood Borne Pathogens Assessment. Additional modules may be required by clinical site. ACE will notify students of the additional required modules.

Professional liability insurance

The College of Nursing will provide liability insurance through Nursing Practice Corporation at a cost of $10.29. Coverage is through the academic year (Aug. 15 through Aug. 14) regardless of when you begin clinicals. Students who begin clinicals in the spring/summer will be required to pay two academic years.
